One audit = one clinical note submitted through the full three-pass AI pipeline (PHI scrub → clinical audit → grade). Resubmitting the same note counts as another audit. Batch uploads count each note individually.
When you reach your plan's monthly audit limit, new audits will be paused until your quota resets at the start of your next billing cycle. There are no overage charges — if you need more audits before the reset, simply upgrade to the next plan. Upgrades take effect immediately.

Every audit checks that your submitted ICD-10 diagnosis codes (1) are appropriate for the documented clinical presentation, (2) are sufficiently specific for CMS standards, and (3) support the medical necessity of the billed E/M level. ICD-10 findings appear as pass/warn/fail items in your compliance report.
